Description of Operations and Guidelines:
Yoga Learning Center and Vitality Yoga & Wellness is providing classes for Yoga, Pranayama (Breathing Techniques), Meditation, physical exercises, Workshops, and misc. events related to the above.
The classes may be video recorded, or photographs taken during the classes for the purpose of but not limited to publicity, illustration, advertisement, web content and social media.
I acknowledge:
I understand that it is my responsibility to consult with a physician prior to and regarding my participation in an in person or online Yoga Class or Workshop. I represent and warrant that I am physically fit and I have no medical condition which would prevent my full participation in a Yoga Class/Workshop.
In consideration of being permitted to participate in an in person or online Yoga Class or Workshop, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in an in person or online Yoga Class or Workshop.
In further consideration of being permitted to participate in an in person or online Yoga Class/ Workshop, I hereby RELEASE, WAIVE, DISCHARGE, INDEMNIFY and HOLD HARMLESS Vitality Yoga & Wellness, its employees, yoga instructors, volunteers and owners, Individually, from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or personal injury, including, but not limited to, illness or death, that may be sustained by me as a result of participating in a Yoga Class/Workshop, including, but not limited to, exposure to the Covid-19 virus or any other infectious diseases.
It is my express intent that this Waiver of Liability and Hold Harmless Agreement shall bind the members of my family and spouse (if any), if I am alive, and my heirs, assignees and personal representative, if I am not alive, and shall be deemed as a RELEASE, WAIVER, AND DISCHARGE of Vitality Yoga & Wellness, its employees, yoga instructors, volunteers and owners, individually. I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be construed in accordance with the laws of the State of New Jersey.
I understand that Vitality Yoga & Wellness, its employees, yoga instructors, volunteers and owners will not be responsible for any medical costs associated with any injury.
I have read the above Waiver of Liability and Hold Harmless Agreement and fully understand and hereby agree to its contents.
If registrant is under 18 a legal guardian's consent is required. As Legal Guardian of Registrant, I consent to the above terms and conditions.
I understand that there shall be no royalties or compensation or payments to me for the photographs and video recordings as described in the description of operations and guidelines.
All cancellations require a 24-hour notice. No refunds will be provided for cancellations after 24 hours or missed classes.
Payments are due before class begins.
Payments are valid for 2 weeks from the date of payment.
